Insurance/ Payments:

 

Up-to-date insurance cards, co-pays and self pay fees are due at time of visit.

Late Policy:

​

 If you are more than 15 minutes late for your appointment, we will have to reschedule your appointment.  

​

Appointments/ Cancellations: 

​

Effective January 1, 2022 there will be a $50 fee for No-Show appointments.

Same day cancellations are also considered No-Shows.

As a courtesy, when time allows, we make reminder calls for appointments at 1 week and/or 1 day prior to your child's appointment. If you do not receive a reminder call or message, the policy will remain in effect.

If you are unable to keep an appointment please notify us at least 24 hours in advance.

 

Physician Coverage:

 

Your primary physician is Kinkee Chung, D.O.  We also have 2 Family Nurse Practitioners on staff. Your appointment may be scheduled with any of these providers depending on your needs and availability.
