Up-to-date insurance cards, co-pays and self pay fees are due at time of visit.

Late Policy:

​

 If you are more than 15 minutes late for your appointment, we will have to reschedule your appointment.  

​

Appointments/ Cancellations: 

​

Effective January 1, 2022 there will be a $50 fee for No-Show appointments.

Same day cancellations are also considered No-Shows.

As a courtesy, when time allows, we make reminder calls for appointments at 1 week and/or 1 day prior to your child's appointment. If you do not receive a reminder call or message, the policy will remain in effect.

If you are unable to keep an appointment please notify us at least 24 hours in advance.

 

Physician Coverage:

 

Each patient in our practice has a primary physician. Routine preventative care and as many sick visits as possible will be with this physician. On days off or during vacations, the other providers in the office will provide coverage. Some preventative and sick visits may be scheduled with our nurse practitioner at your discretion.
